What '6% of received claims' means
You are billed on money that actually posted, not on charges, not on billed charges, not on a monthly minimum that ignores a slow OR week. If a claim is denied and we have not collected it, it is not in the percentage.
Volume can move the rate down. We will say so up front instead of burying a 'promotional' number that expires.
