Independent CRNA billing (QZ)
Modifier QZ means the CRNA performed the case without medical direction. That is typically 100% of the allowed amount to the CRNA or the CRNA group. A general biller who copied a hospital template will hang QX on it and split a fee you never should have split.
If your practice is medically directed, we are not the biller. Our book is QZ (and AA on the physician side). State opt-out status and facility bylaws still matter for whether QZ is allowed. That is intake, not a footnote after the first denial.
MAC cases (QS)
Endoscopy, ophthalmology, and other MAC lists still need start and stop time, the right base units, and QS on the claim when the service is monitored anesthesia care. QS is informational. It rides with QZ. Skipping it, or putting it on a general anesthetic, is how MAC lists get denied.
What CRNA groups usually lose money on
QZ billed as QX. Labor and OB time billed like a 30-minute MAC. Physical status left off. Relief and lunch breaks that break continuous time. Payer enrollment that still lists an old collaborating physician. Those are CRNA problems, not generic A/R problems.

What is modifier QZ in CRNA billing?
QZ reports CRNA service without medical direction. The CRNA (or group) bills the full allowed amount. We do not bill the directed pair (QX with QK or QY).
Do you bill medical direction for CRNAs?
No. We bill AA, QZ, and QS only. If your group runs medically directed rooms, we will tell you that on the first call.
Can you credential new CRNAs?
Yes. Provider enrollment and revalidation are part of the same firm that bills the claims, so a new hire is not waiting on a separate vendor.
